There have been some changes to the way entry clinics will be handled effective immediately.

  1. Requesting materials for a new clinic
    Use the "Clinic Request" form. This is pretty much the same as last year, with a couple of minor changes. Send to both Jose Cordova, SRA (for shipping of materials) and Dave Busekist, SDI (for approval and web posting).
  2. Collecting data for new referees
    This is the most significant change. You are no longer required to use the traditional USSF individual registration forms. Instead, the data that used to be entered on the USSF form will now be collected in a single spreadsheet (see "Registration" form). At the end of the clinic, the completed form will be sent electronically to The SRA at LouisianaSRA@gmail.com. It is extremely important for this form to be completed accurately for each referee, as this information will be entered in this exact form in the USSF database. The USSF form used for collecting the data is a good place to start. The new form is more efficient and less prone to spelling errors, etc. Allowing each new referee enter it directly (if you have a laptop on site, for example) would allow you to print a copy and have it reviewed for accuracy. Do whatever works best for you, but please do not submit individual USSF registration forms.
  3. Submitting test scores and fees As done in past years, instructors will collect the individual graded test answer sheets and registration fees ($40 per referee) and send them to the SRA. Please try to avoid sending cash (if people give you cash, it is suggested that you write a check for the total cash amount and keep the cash or send a money order and keep the cash). Everyone's registration gets delayed if a discrepancy between what is submitted and what should have been submitted exists. Please discard any old answer sheets you may have used in the past and use the new answer sheet. The revised answer sheet has a place for the referee to sign acknowledging some USSF language. This sheet (and the money) is the only thing you need to submit to the SRA after the clinic.
  4. Risk management
    Please don't send these to the SRA. It creates unnecessary delays.
    All minors (17 and under) should give you a properly completed Risk Management form signed by their parents. Give them the form on the first day of the clinic and hopefully they will bring it back on the second day. Send these forms to LSA as indicated on the form. Adults should do their Risk Management online. If an adult has already completed a current soccer year disclosure as a coach, manager or volunteer they do not need to complete another one as a referee. The other form applies -- duplicate forms slows the process. Incomplete/Nonexistent Risk Management disclosures are by far the most common reason for delays in registrations.

This form must be submitted by the instructor of record to the SRA to complete the registration process for those who successfully completed the clinic.

This answer sheet sheet has the disclaimer notice present on the standard USSF registration form. Used in conjunction with the post-clinic Registration Form (now required) it will not be necessary to include the USSF registration form with payment when registering new referees.

This form is necessary for all minors (17 years of age or younger). Adults can complete the online version an the LSA website (www.lsa-soccer.org under Risk Management). Instructors may collect the forms from minors and send to LSA. Please make sure prospective referees know they are not registered until this form is received and approved in the LSA office.
